It would indeed be nice to have it online. I don't really have time to
do that this year, but we should consider it for the future. In any
case, it would be nice to have a better plan for the evaluation of the
conference, something to bring up at the next meeting...

In the mean time the best I have to offer is the pdf of the paper form.
If they can return it to me completed, by snail-mail or email, I'll make
sure to include it with the others.
